International travel procedures for group student travel

The guide below is intended for faculty and staff who are traveling internationally with students (undergraduate or graduate). Please review each step and submit all completed forms within 30 days of the departure date.

Step 1 (to be completed by faculty/staff member)

Complete the International Travel Authorization Request Form (Form 1) for yourself. Once completed, your form needs to be signed by the Dean or department chair prior to uploading into Workday as an attachment to the spend authorization. Please make sure to list all students traveling with in the section titled "If traveling with others, please list names/affiliates."

Step 2 (to be completed by faculty/staff member)

Complete the Faculty-Led International Travel Safety and Risk Assessment Form (Form 2). Once completed, your form needs to be signed by the Dean or department chair prior to uploading into Workday as an attachment to the spend authorization. Only one risk assessment needs to be completed for the entire group traveling.

Step 3 (to be completed by student)

Each individual student is required to complete items 3-9 below and submit them via email to BCNRisk@unr.edu:

  1. Student Agreement for International Travel (Form 3) This form must be submitted with all pages and have a wet signature. Forms missing pages or signed electronically/digitally will be returned for completion.
  2. Emergency Information Sheet for International Travel (Form 4)
  3. Waiver, Release, and Indemnification Agreement for International Travel (Form 5) This form must be submitted with all pages and have a wet signature. Forms missing pages or signed electronically/digitally will be returned for completion.
  4. International Travel Health and Safety Clearance Packet (Form 6)
  5. International Travel Disability Accommodation Request (only complete if applicable Form 7)
  6. Provide a copy of Travel Insurance*
  7. Provide a copy of your passport

*Students are required to purchase their own travel insurance through a group policy at CISI sponsored by the University. Please use this link to the group travel insurance policy. Graduate students who are enrolled in the graduate school health insurance program may provide a copy of their health insurance card in lieu of purchasing the CISI group policy as proof of travel insurance.
