International students and scholars can use the forms below for admissions, training, immunizations, shipping and more. Please be aware it may take the Office of International Students and Scholars time to process your forms after receiving them at our office. We appreciate your patience and cooperation.

- 12-Month OPT application form. If you started an application, login with your NetID to international.unr.edu to complete it under "Records".
- 12-Month OPT update form
- 24-Month STEM OPT application
- 24-Month STEM OPT update form. Form for students on STEM OPT to report any changes in their employment situation to the OISS. A new Form I-983 for the new employer and a Form I-983 final evaluation from the previous employer must be submitted.
- 24-Month STEM OPT validation report. Validation report that should be submitted every 6 months throughout the STEM OPT extension
