Annual Survey Instructions

Thanks for participating in this important research project.

As part of the study you are requested to complete the LEND Trainee Follow Up Survey annually between February-April for each year of the study. The survey should take no more than 10 minutes, and the information is strictly confidential and securely stored. A sample of the Sample Trainee Follow Up Survey is provided.

Brief instructions

  1. Click on the “NvLEND Trainee Followup Survey link“
  2. Select the NV-Nevada Center for Excellence in Disabilities from the drop-down list
  3. Enter in your provided login.
  4. Press “Submit”.
  5. Complete the Trainee Survey—it should take at most 10 minutes.
