Camp agreement and required forms

All campers are required to adhere to the camp rules and 4-H Camp policies. They are expected to participate in all camp activities as specified in the handbook. Failure to follow rules and meet participation expectations may result in dismissal from camp without a refund of camp fees. If an incident occurs that warrants the student's dismissal from camp, parents are required to provide transportation home in a timely manner.

Required forms to complete registration

The Camp Handbook contains rules and requirements campers must adhere to at camp, as well as detailed information about the camp. Please read the Camp Handbook before registering. A waiver is acknowledged during online registration at checkout.

The camp agreement and health form are due within two weeks after completing the online portion of the registration. These forms are online submissions and you will receive an email confirmation of each submission for your records.

At check-in, campers and parents/guardians must meet with camp staff to review the Health form and to discuss any medications and/or health conditions that may affect the camper's experience.

Optional arrival/departure form

This form is only for campers driving themselves to camp or arriving by plane, taxi or ride share without a parent/legal guardian. This form must be submitted May 31.
