Camp agreement

All campers are required to adhere to camp rules and 4-H Camp policies. They are expected to participate in all camp activities as specified in the camp handbook. Failure to follow rules and meet participation expectations may result in dismissal from camp without a refund of camp fees. If an incident occurs that warrants the student's dismissal from camp, parents are required to provide transportation home in a timely manner.

Required forms to complete registration

Camp agreement, health form, and waiver, release, and indemnification agreement are due within 10 business days after completing the online portion of the registration. These required forms must be mailed or faxed to the camp office. (For best results for the fillable PDF forms, please use Chrome web browser or download to complete forms electronically.)

Please note: If these forms have not been received within 10 business days of completing online registration, your registration will be canceled and your money will be refunded.*

* For those campers arriving by plane or driving their own vehicles, please complete the arrival/departure form, which is to be submitted to the Lake Tahoe Music Camp office no later than June 1, 2019.

Camp Agreement Signature Page

This agreement is required for all campers.

The camp agreement contains rules and requirements campers must adhere to at camp, as well as detailed information about the camp. Print, sign and submit only the last page of the camp agreement.

Health Form

This form is required for all campers.

At registration, campers and parents/guardians must meet with the assistant camp director to review this form and to discuss any medications and/or health conditions that may affect the camper's experience.

This agreement is required for all campers.

Arrival/Departure Form

This form is required ONLY for those campers arriving by plane or driving themselves to camp.
