Flowchart: The IBC Review and Approval Process

The image above shows the steps for completing the University's IBC review and approval process. Below are the same steps in detail for both the MOUA (biological use protocol) and the MOUA amendment.

MOUA (also known as a biological use protocol)

Principal investigator (PI) downloads MOUA form (also known as a biological use protocol). Note – there are 3 different type of MOUA forms to choose from:

  1. MOUAs/MOUA amendments must be submitted as a Word document to Kristin Eliasen (keliasen@unr.edu ). A lab map must be included with the submission
  2. The principal investigator/CEO needs to sign the front page of the completed MOUA application then obtain the department chair/NCAR director’s signature. NCAR director signature is required for private businesses and this signature can also be obtained by EH&S personnel.
  3. The MOUA must be provided to the IBC at least 10 business days prior to the next scheduled IBC meeting. Protocols received after this deadline will be reviewed at the next scheduled IBC meeting. The IBC meeting schedule can be found on the IBC website or on the UNR calendar. Links are provided below.

  4. After receipt, the MOUA is sent to the IBC members for a preliminary review. Members provide comments on the protocol that are then sent to the PI. The PI is asked to submit a revised protocol that addresses each of the IBC comments.
  5. The revised MOUA is sent to the IBC for a second review, if there is sufficient time before the IBC meeting.
  6. Protocols that are reviewed at the IBC meeting and determined to need only MINOR REVISION may be “approved, contingent on the IBC comments being addressed in a revised MOUA.” In this instance, the PI/CEO must address the remaining or additional IBC comments prior to the MOUA receiving full approval. Once the IBC comments are adequately addressed, the IBC Chair will provide the final signature approval (see section 1f.1).
  7. Protocols that have addressed all of the IBC comments are approved “as submitted”, unless additional concerns are identified by the committee.
    1. Once all IBC comments have been addressed, the MOUA is provided to the IBC Chair for final signature approval.
    2. After EH&S receives formal approval of the MOUA from the IBC chair, notification of the approval is sent to the Office of Sponsored Projects and the PI/CEO who submitted the MOUA. Note – notice of MOUA amendment approvals are sent only to the PI/CEO.
  8. Protocols that need MAJOR REVISION are sent to the PI/CEO to address the additional IBC comments. If major revisions are needed, the PI/CEO will be asked to make the necessary revisions and will be notified that the MOUA will be reviewed at a later date, which might not be until the next IBC meeting if in-person committee review is required. Once the PI/CEO provides a revised MOUA, the submittal will start the review process for a second time (see section 1b).

MOUA Amendment
