Claim Procedures
After discovery of a loss or a situation that may result in a loss:
- Notify appropriate authority (police, fire, etc.) as soon as possible.
- Notify the Risk Management Department (775) 784-6139 as soon as possible.
- Complete form URM-013, a Crime and Fidelity Loss Report.
- In the event there is reason to believe the loss is due to employee dishonesty, the department should also notify the human resources office and internal audit immediately. You will be provided with further instructions as to how to proceed.
Discovery of Loss occurs when you first become aware of facts which would cause a reasonable person to assume that a loss has been or will be incurred; even though the exact amount or details of the loss may not then be known.
Failure to report the loss or a situation that may result in a loss, in a timely manner, may result in the claim being denied.
The Risk Management Department will request the following as part of their investigation and reconciliation: the appropriate authority's report (police, etc.); a reference to the claimants' fund reimbursement number, and claim cost documentation (i.e.: invoices, purchase orders, estimates for repair, inventory records, etc.).
Conditions
There is no coverage for an employee immediately upon discovery of any dishonest act committed by that employee (whether before or after becoming employed by the Institution).
Loss Control
The Risk Management Department should be notified even if an employee reimburses the University and there is no actual claim being made.
