Health Fairs

The Public Employees' Benefits Program (PEBP) offers Health Fairs for participants in the Self-Funded PPO Plan (currently employed or retired) and their covered spouses and dependents 18 years and older.

You must show your State of Nevada PEBP insurance card with new member ID number for entry to the Health Fair. The fairs are designed to provide Self-Funded PPO Plan participants and covered dependents with convenient health screenings and materials related to healthy living.

NOTE: Anthem HMO Nevada participants do not participate in this Health Fair.

Health Insurance

Employees may choose between several insurance options, depending on what part of the State they live in.

Dependents may be enrolled for insurance within the first 60 days of employment. Dependents may be added after the 60 day period only during an annual open enrollment period or within 30 days of a qualified change.

Qualified changes include marriage, divorce, death, birth/adoption, change of spouse's employment status, and involuntary loss of insurance coverage. Proof of the qualified change must be provided.

Once a year, the Public Employees' Benefits Program holds an open enrollment period, during which employees can change medical the medical plan they are enrolled in, and add or drop eligible dependents from their health insurance. The effective date of any change would be July 1.

This document provides definitions of specific terms and words as used in the Public Employees’ Benefit Program (PEBP) Plan Document, or that would be helpful in understanding covered or excluded health care services. These definitions do not, and should not be interpreted to, extend coverage under the Plan.   Additional definitions and the complete plan documents can be found at ww.pebp.state.nv.us.

Continuation of Health Insurance at Termination of Employment

In compliance with a federal law commonly called COBRA (Consolidated Omnibus Budget Reconciliation Act), this Plan offers its employees, retirees and their covered Dependents (called "Qualified Beneficiaries" by the law) the opportunity to elect a temporary continuation ("COBRA Continuation Coverage") of the group health coverage sponsored by PEBP, including medical coverage, when that coverage would otherwise end because of certain events (called "Qualifying Events" by the law). The participant must be covered by the group health coverage sponsored by PEBP the day before the Qualifying Event in order to continue coverage under COBRA. Qualified Beneficiaries who elect COBRA Continuation Coverage must pay for it at their own expense. Additional definitions and the complete plan documents can be found at ww.pebp.state.nv.us.

Life/Accident/Long-Term Disability

Benefits eligible employees are provided with:

  • $20,000 in life insurance
  • $20,000 in personal accident insurance
  • $50,000 in business accident insurance
  • Long-Term Disability insurance provides approximately 60% of the employee's gross monthly income, up to $7,500, after 180 days of total disability.

Supplemental Life Insurance

The program will provide up to $300,000 guarantee issue coverage limited to four times salary. The first $150,000 is guarantee issue regardless of salary for the eligible employee and $50,000 guarantee issue coverage for the spouse/domestic partner. The spouse/domestic partner guarantee issue is limited to 50% of the employees amount to a maximum of $50,000. The guarantee issue is without having to submit evidence of insurability, provided they apply during the first 60 days of first being offered this program. Dependent children may be insured up to a maximum of $20,000 as long as either the employee or the spouse is also covered.  (Guarantee issue = no medical questions; you can’t be turned down).  

If additional term-life insurance is desired, active employees may apply for life insurance up to a maximum of $500,000 for the employee and $250,000 for the spouse/domestic partner.

Coverage will become effective for guarantee issue on the first day of the month following payroll deduction. Coverage for higher amounts will become effective on the first of the month following approval by the insurance company.

Personal Accident Insurance

This policy covers accidental death or dismemberment, permanent and total disability, or paralysis. Eligible employees under age 70 may enroll in this plan at any time without having to provide evidence of insurability. Employee Only and Family Plans are available. The effective date is the first of the month following the payroll deduction.

Short-Term Disability Insurance

This plan pays a monthly benefit if you become totally disabled on a short-term basis. Benefits begin on the 15th day of disability and pay up to 26 weeks or to age 70, whichever occurs first. If approved, the effective date is the first of the month following the payroll deduction.

Cancer Care Insurance

This plan is available for employees under the age of 65 who have not previously been diagnosed as having cancer. Employee Only and Family Plans are available. A two year waiting period for coverage is required if cancer is diagnosed within 30 days after the date of issue. The effective date is the first of the month following the payroll deduction.

Personal Recovery Plus

This program provides financial assistance if you are faced with expensive medical treatments and loss of income for certain specified health events. These specified events include: heart attack, stroke, coronary artery bypass surgery, coma, paralysis, major third degree burns, end-stage renal failure, and major human organ transplant. AFLAC pays the plan benefits directly to you so that you can determine the best use for this additional income.

Your age and the level of coverage you select determine the costs of this plan. Premiums (costs) will be withheld from your pay on a before-tax basis.

Voluntary Indemnity Plan

Under this program, AFLAC will pay benefits for daily hospital confinement, for illness or for accident-related events. Additional benefits are payable when you have a surgical procedure. Ambulance benefits are also payable. Cash benefits are paid directly to you.

Long-Term Care

This program provides you with assistance if you have a chronic condition that leaves you unable to care for yourself over an extended period of time. Long-term care may be received in your home, in an adult day care center, in an assisted living facility, a nursing home or in other locations. This policy generally provides benefits for expenses not covered by Medicare or your health insurance plan.

Your age and the level of coverage you select determine the cost of this plan. You may apply for participation in this plan at anytime during the year. Premiums (costs) for the long-term care plan are withheld from your pay on an after-tax basis.

Section 125 - Flexible Benefit Plan

The Section 125 - Flexible Benefit Plan offers you the opportunity to pay for the following benefits on a tax sheltered basis: Health insurance premiums for dependent coverage, personal accident insurance premiums, cancer care insurance premiums, personal recovery plus, and voluntary indemnity plans.

Also available under this plan, you can establish a medical reimbursement expense account, and/or dependent care reimbursement account. If you do not file sufficient claims for reimbursement under these two accounts, you will lose the unused amounts. This is often referred to as the "use it or lose it" rule.

You must sign up for this program within the first 60 days of employment. There will be an open enrollment period in the spring, generally in May. During this open enrollment, employees may enroll in the Section 125 plan for the next fiscal year. Employees wishing to continue participation in either of the reimbursement accounts must re-enroll every year during the open enrollment period. There is no allowance for late enrollment.

When payroll deductions are taken on a before-tax basis, your taxable earnings are reduced by the amount of these deductions for the purpose of computing your taxes. Because of this tax advantage, federal law restricts employees from making changes in their benefit elections except during the open enrollment period, to be effective the following July 1.

THIS MEANS YOU CANNOT ADD, DROP OR CHANGE YOUR BENEFIT ELECTIONS AT ANY OTHER TIME EXCEPT IN CASES OF:

1. Change in family status such as marriage, birth, divorce, or death.
2. Inadvertent loss of coverage due to loss of employment, age, etc.

This plan is available to all full-time active and retired employees, their spouses, and the employee's parents and parents-in-law to help offset the financial burden of long term care. Examples of long term care are nursing home care, community based care (home health, adult day care, homemaker services) or respite care (relief for the care giver).
