The university relies on all faculty, staff and students to help protect the university's assets and reputation. You may have concerns about an activity without necessarily having proof. If you suspect fraud, misuse, misappropriation or any other improper activity, please complete the following form and email it directly to Campus Audit.

Information about your identity is optional. None of the fields below requires an entry. Your browser may highlight some fields, do not consider these required fields. Once you complete and submit this form, it will be emailed directly to one of the Campus Auditors. All information provided will be confidential. Please understand, however, that the effectiveness of our investigation is contingent upon the completeness and accuracy of the details provided to us. If you choose to remain anonymous, we will be unable to ask you any questions or discuss the situation with you.

Reports related to other areas of governance (i.e. Human Resources, Research, Police) will be forwarded to that department for further review in accordance with their respective policies and procedures.
