Because we have received an unprecedented number of applications, only those applications submitted prior to July 15 will be reviewed for the fall 2009 semester. Applications received after July 15th will be held for review until such time as more slots become available in our program.

Step 2: Eligibility Verification/Parent & Student Signature Page
When requested, you will need to provide a signed original (faxes accepted).

 

Step 3: Visit IRS Page 1 & 2, EXAMPLE U.S. Individual Income Tax Return/1040a or 1040ez

When requested, you will need to provide a copy of 1040a side 1 & 2 ONLY.

The signature page and tax documents (yes copies are okay, but please have them signed) can be turned in to our office (Thompson Building, Suite 200) or faxed to (775) 784-1353.
