Medical Records
To request a copy of your medical record, you must fill out and sign a medical records release form.

Print a copy of the medical records release form, fill it out, sign it and fax it to (775) 784-1298 or send it to University of Nevada, Reno Student Health Center, Mail Stop 196, Reno, NV 89557.
There is no charge for one or two page copies (immunization record, lab result). We can mail or fax the information to you. For medical records sent to another physician, there is no charge.
We cannot fax your entire medical record. If you want a copy of your entire medical record for personal use, there is a charge of 60 cents per page. We will mail you the information or you can pick it up at the UNR Student Health Center. Checks should be made payable to the Board of Regents.

Please include the following information in a request for medical records:

  • Name
  • Birthdate
  • Complete address and phone/fax number
  • The last semester you were seen at the UNR Student Health Center

We keep medical records for 7 (seven) years after a student is last seen at the UNR Student Health Center. The records are then destroyed. You may request a copy of your medical record anytime prior to the seven-year deadline.
