There are many important forms and paperwork the university requires undergraduate students to complete, including the Declare/Change Major Form, the Minor Program Form, the Field of Concentration Form, etc. Now, instead of coming all the way to the Psychology Department to get the forms, you can download them to your computer and print them out!


Change of Major/Additional Major/Degree Declaration Form

This form is used for the following functions:
1. Declaring your major (if you had not already done so when applying for admission to UNR)
2. Changing your major
3. Adding another major (to pursue a Dual Degree or Dual Major)

Psychology Major Bachelor of Arts Checklist - Graduate School Preparation Track

This handy checklist allows you to note your progress through all of the requirements (including those of the university, college, and department) for a Bachelor of Arts degree in Psychology. This version is for students who entered the major in Fall 1997 or after and are pursuing the "Graduate School Preparation Track" Psychology major.

Psychology Major Bachelor of Arts Checklist - Liberal Arts Track

This handy checklist allows you to note your progress through all of the requirements (including those of the university, college, and department) for a Bachelor of Arts degree in Psychology. This version is for students who entered the major in Fall 1997 or after and are pursuing the "Liberal Arts Track" Psychology major.

Psychology Minor Form

This form is similar to the field of concentration form, but is for the minor program in Psychology. This version is for students who entered the minor Fall 1997 or after.

Had a meeting with a Psychology Department advisor recently? You can use this form to provide an evaluation of the quality of the advisement and information you received. Simply fill out the form and then return or mail to the Psychology Department main office (MSS 438).
