          Public Records Request Form  EXCEL

In accordance with Nevada System of Higher Education Procedures and Guidelines Manual, Chapter 4, Section 18, approved April 29, 2014, the University is allowed to charge for public records at $.10 per page after the first twenty (20) pages.  If responding to the request requires an extraordinary use of personnel or technological resources, the University may charge an additional reasonable fee. The fee charged for the extraordinary use of personnel or technological resources will be based upon the actual cost incurred in responding to the request, but will not exceed $.50 per page. Extraordinary use of personnel resources means an excess of two (2) hours of employee time expended in responding to the request. The fee charged for the extraordinary use of personnel resources will take into consideration, but not necessarily be limited to, the gross hourly wages of staff member(s) involved in responding to the request. Any such charge will be actual time required to respond to the request, but will not include the first (2) hours. All estimated fees are to be paid in advance and if the actual cost is less than the estimate, the balance will be returned to the requestor.  For further information please review Nevada System of Higher Education Procedures and Guidelines Manual, Chapter 4, Section 18. PDF
