The State of Nevada Public Employees Benefits Program (PEBP) Board approved the following plan design changes for Plan Year 2015, effective July 1, 2014
Consumer Driven Health Plan
The coinsurance rate on the CDHP will change from the current 75% (Plan) and 25% (participant) to 80% (Plan) and 20% (participant) after the plan year deductible is satisfied.
The Plan will cover one annual preventive vision screening exam paid at 100% under the wellness benefit.
The CHDP deductible will be reduced from $1,900 to $1,500 for individual coverage (participant only) and from $3,800 to $3,000 for family coverage (participant plus one or more covered family members).
HMO Plan
Co-payment for Outpatient Surgery will change from the current $1,000 to $350
Co-payment for Inpatient Hospitalization will change from $1,500 to $500
Dental Plan
The annual dental maximum per covered member will increase from the current $1,000 to $1,500 per covered member.
Health Savings Account (HSA) and Health Reimbursement Arrangement (HRA)
Participant only: $700 (base) + $400 (additional contribution for employees enrolled effective July 1, 2014) = $1100
Dependent (up to 3): $200 (base) + $100 (additional contribution for dependents enrolled effective July 1, 2014) = $300
The HRA for CDHP participants will continue to accumulate without an annual or lifetime limit (fund balance will carry over from the current plan year to the next plan year).
The HRA for Medicare retirees enrolled in medical coverage through the Medicare Exchange will continue to accumulate without an annual or lifetime limit (fund balance will carry over from this current plan year to the next plan year).
Life Insurance
Increased basic life insurance from $10,000 to $25,000 for active members and from $5,000 to $12,500 for retirees.
NVision Health & Wellness Program
PEBP will continue the NVision Health & Wellness Program and its associated premium incentive (up to $50/month) for CDHP and HMO primary participants who complete the required steps to earn premium credit for Plan Year 2015, effective July 1, 2014.
The PEBP Open Enrollment period starts May 1 and ends May 31. Open Enrollment is an employee's one chance during the year to evaluate benefits and review plan design changes that will occur for the next plan year, and make changes if necessary. Only employees who wish to change health insurance option or add/delete depends are required to take action. Any changes to coverage should be processed through the PEBP on-line e-PEBP Enrollment System which is available 24/7 from May 1 through May 31, 2014. Any changes will become effective July 1, 2014.
