Students should contact their major department or school as early as possible to arrange for an academic advisor and should meet with their advisors regularly, especially before registering for courses. School phone numbers are listed on the Programs webpage and in the University Directory.

Every student must complete an application for graduation which also requires a fee. There are deadlines for each semester. The required application and additional information can be found online on the University website.
128 All students must complete at least 128 semester college credits. Even if a student completes all the required courses in major, 128 credits is a requirement of graduation.
64 At least 64 of those credits must be earned at a 4-year institution.
40 At least 40 upper-division credits must be earned at the Junior –Senior level (courses numbered 300 or higher). Please note some majors (Nursing) require more than 40.

Students are responsible for keeping track of their progress throughout the core curriculum. It is strongly recommended that students complete lower division Core Curriculum requirements by the beginning of their junior year. Requests for substitutions, waivers and exceptions must be made before the senior year. It is also strongly recommended that students meet with their academic advisor each semester before registering for classes.
In order to change your major to a choice in the Division of Health Sciences, you will need to download a Change of Major form from Records & Registration, visit the Division office, complete the form, request the vice president's signature, then take it to Admissions and Records. Please plan on meeting with the advisor in your new major to see how your completed classes meet the requirements of your newly declared major.
Another form you may need to use is the Repeat Adjustment Request form. Please consult with an advisor to make sure that you are getting the most out of this process.
If you have been put on “Disqualification” status, please plan on meeting with the Division of Health Sciences Coordinator for Advisement, Recruitment and Retention for Advisement. During that meeting you will obtain a signed form that you will take to Admissions and Records in order to register for classes. This is the ONLY way you will be allowed to register.
