Links on this page provide a variety of types of forms and information for program participants and their parents. The final link leads to discussion areas where girls (in two separate areas) and their parents (combined in their own forum) can discuss common interests and issues within designated guidelines.

Forms for Registrants

Girls who have been accepted into the program and their parents should complete the following forms and return them to: Program Assistant, Girls Math & Technology Program; College of Education/MS 299; University of Nevada, Reno; Reno, NV 89557.

• Registration Form (for accepted applicants)
  Online and downloadable registration forms for girls who have been accepted into the program.
• Participation Agreement
  Required form regarding expected summer camp behaviors.
• Consent Forms
  Parent/daughter permission forms for participating in the research aspect of this program.

    (Note: Completion of the third form is required for participating in the electronic discussion areas.)

  Downloadable letter for individuals to use in seeking sponsors to fund their registration fee.

  Participant options available during designated late-afternoon and early-evening spare-time segments during the summer camp.

Discussion Areas

Separate discussion areas for each group of girls and a joint discussion area for parents; discussion must fall within designated guidelines. Participants may read and add comments at any time without being online simultaneously. Completion of Electronic Bulletin Board form required for participation.
