Practicum Students
THIS FORM DOES NOT HAVE TO BE DONE IN ORDER FOR YOU TO REGISTER.
IF YOU CAN NOT REGISTER, PLEASE CONTACT YOUR DEPARTMENT.
This process is required by WCSD.
Once you have been scanned and have filled out the Volunteer forms, your information will be valid for 1 year.
For ALL Faculty & Staff, you MUST use this form from this LINK. (This is not the form for your students)
For ALL Practicum Students, you MUST print out the following forms from this LINK.
Fill out the 2nd and 3rd pages of the packet.
Please bring in the following information with your class as soon as they are scheduled to
be scanned
Bring the packet and an original Government issued photo ID to the Office of Field Experiences - WRB, rm 2004.
For foriegn students, please bring your passport.
This process MUST be done within the first four weeks of the semester or you will not be
allowed out into the schools.
