DOCUMENTATION TIMELINE

 

Weeks 1-2:

Download, read and provide a copy to your lead teacher and university supervisor:

  • General Policies Guide
  • Appropriate program manual guide
    In your appropriate program guide II-VI, pay particular attention to the section called: Focus for observations of lead teacher with follow-up practice.
  • Student Learning Sample Assignment (SLS) – begin first section now (See your program guide for more information)
  • Student Learning Sample meetings are MANDATORY


  • If you have not taken the Praxis II, register for it now! It takes several weeks to get the results and testing sessions fill up fast.
  • Bookmark the web bulletin board for easy access, check weekly and read messages. This is how the Director communicates with you.

    Complete the following forms, have your lead teacher and university supervisor check them and then put them in your Portfolio II:

Set up first meeting with lead teacher and supervisor and have the following forms available:

Procedures for Initial Meeting with Intern -Print or get a copy from your Supervisor

  • Set up the classroom computer with the Lead Teacher Orientation and ask your lead teacher to review it. Have your lead teacher complete the appropriate teacher information page and send it to the Director.
  • Set up your intern notebook and Portfolio II with appropriate sections. These can also be a series of folders-you dont have to buy notebooks for this purpose. The key here is organization.

Before midpoint:

2 formal observations by supervisor and 2 formal observations by lead teacher

  • Remember NO PLANS, NO TEACH! Examine the lesson plan format in the SLS manual.
  • Complete the Intern’s Pre-Observation Information for Supervisor form (this can also be used for your lead teacher) for each formal observation.
  • Print (or get a copy from your Supervisor) the Classroom Observation form (pilot F06) for each formal observation. Have extra copies handy for impromptu observations. Your lead teacher and university supervisor will use this form for their formal observations.
    Music interns (must print out) will need to use - General Music Classroom - Music Rehearsal Classroom
  • Attend all seminars and meetings as required.
  • You should be gradually assuming the teacher’s responsibilities and have layered on full control of about 1/2 the school day by midpoint.
  • Begin observations of other classes. These are for a class period or instructional block only, not an entire day.

Midpoint in internship:

  • Schedule your midpoint progress conference. Make sure it is a three-way conference with you, your lead teacher and supervisor-yes, this is necessary.
  • Print (or get a copy from your Supervisor) of the Midpoint Progress Report for use at the conference.
  • After the conference, fax the signed Midpoint Progress Report to the Director at (775)327-2323 and put in Portfolio II.
  • Attendance at Director's meetings & SLS meetings are MANDATORY.

After midpoint:

3 formal observations by supervisor and 3 formal observations by lead teacher

  • Attend all seminars and meetings as required.
  • Finish Student Learning Sample- it must be graded before your internship ends.
  • You should continue assuming the teacher’s responsibilities until you have full control of the entire school day. Complete a minimum of 4-weeks full control or longer.
  • Your supervisor and lts should discuss your final evaluation during week 14 (Duals weeks 8 and 18)
  • Schedule your final evaluation conference for next week. Make sure it is a three-way conference with you, your lead teacher and supervisor.
  • Print and have your lead teacher and supervisor complete the Professional Behavior and Dispositions evaluation. Put these in your notebook.

Next to last week of internship:

  • Intern, LT, & Supervisor must complete & submit evaluations of each each other by filling out the PDF evaluation form (located below).
  • Evaluation of:
  • Have your Notebook ready for review and accessible to your lead teacher and university supervisor.
  • Write your self-reflection essay now. Put a copy in your notebook.
  • Supervisor will enter the evaluations from the lead teacher(s) and themselves on one final evaluation form.
  • Supervisor will bring copies to the three-way confrence.
  • three-way conference (intern, LTs and Supervisor) held to discuss intern's final evaluation for internship.
  • Final Documentation to be turned in this week.

Last week of internship:

FINAL EVALUATION (effective immediately) :

All interns: The internship is 16-weeks (Duals have two 10-week internships)and the intern is expected to either be (a) teaching, (b) assisting the lead teacher, or (c) observing in other classrooms through the last day of internship.

  • Given the intern's satisfactory performance through week 15 (Duals weeks 9 and 19), the same level of performance will be expected of the intern for the remaining days of internship in the Five Domains of Professional Competence (see - online evaluation) and their professional behavior and dispositions (see forms). The lead teacher and/or supervisor should notify the Director of Field Experiences immediately if the intern's performance if this changes.
