Important Dates

  • October 12, 2007 - End of Campaign -
  •  October 18, 2007 – Last day for pledge forms to be returned to Denise Madole (M/s 238) and be eligible for the two drawings.  Two tickets on Southwest Airlines or airfare, hotel and game tickets for two to see the Wolf Pack play New Mexico State on November 2. The minimum donation to be eligible for the drawing is $10.00.  .

The steps below will help guide employees in filling out the pledge form correctly. If you have any questions please contact Tim McFarling, Denise Madole, or Kim Bonnenfant  @ 784-6082.

Section 1:

Donor Information - Filling in this information is optional. This will allow the donor to receive periodic updates on how their contributions are making an impact in our community.

Section 2:

Method of Payment - in this section the donor will indicate how they would like to make their gift. The choices are:

  • Payroll Deduction: This is an equal amount for each pay period throughout all of 2008.  There are no one time only gifts with payroll deduction. Please make sure that the total dollar amount of the year’s payroll deduction dollar amount is noted. Also make sure that if the employee has selected payroll deduction that  the pledge form has been signed.
  • One Time Gift
    • Cash or Check: Staple the cash or check to the pledge form.
    • Credit Card Payment, please make sure all information is completed and correct and the employee has signed the form.
    • Is this related to credit card or payroll deduction? If per pay period is selected, the number of pay periods is very important. This payroll deduction will begin in January, 2008 and continue though December 31, 2008.

Section 3:

Targeted Investments - The donor may select either United Way of Northern Nevada and the Sierra or Community Health Charities of Nevada. The option is also available for the donor to designate their gift to an agency of their choice; it must be a 501 (c) (3) Not-For-Profit agency.  For more information on these two charities please refer to their web sites

Important items to remember 

Please return all pledge forms to Denise Madole in the Human Resource department at Mail Stop 0238 or drop them off in the Human Resource Department in Room 1 of the Artemisia Building. 

If the employee has chosen to pay by payroll deduction or credit card check to make sure they have signed the form on the appropriate signature line. 

If the employee is planning on claiming this donation on their 2007 tax forms they will need to retain a copy of the form for their tax records.

In order for your employees to be eligible for the prize drawings please have their pledge forms back to Denise Madole by October 18, 2007 the drawing will be held on October 19, 2007.   The minimum donation to be eligible for the drawing is $10.00.

Campaign Incentives

Individuals

Employees that make a pledge of $10 or more will be eligible to win one of two great travel prizes.

Grand Prize: 2 round trip tickets on Southwest Airlines.

1st Place Prize: Trip for two to Las Cruces, New Mexico to see the Wolf Pack play New Mexico State on November 2, 2007(includes air and ground transportation, game tickets, and a hotel stay.)

To be eligible for both drawings you must have your completed pledge form returned to your department coordinator by Friday, October 12. (A minimum pledge of $10 is required.) The drawing for the prizes will be on Friday, October 19 and winners will be notified on Monday, October 22.

Departments

This year, three departments on campus that achieve 100% participation will be awarded with a ticket for each employee to watch the home football game against Hawaii on November 16, 2007. One ticket package will be raffled off to departments in each of the three categories below:

Small:               1 to 10

Medium:           11 to 30

Large:               31 and up

Participation will be considered as any amount of monetary donation given.  If more than one department in a group has 100% participation there will be a drawing for the lunch prize.
