Program Assessment Plan Template
This is a facsimile of the of the web interface Assessment Coordinators will use to enter, save, and submit their program assessment plans on the OUA web site. Text for any sections of the plan may be entered and saved until the entire plan is complete. Upon completion, the plan may be submitted electronically. The Assessment Office will review each program’s assessment plan and make comments and suggestions as appropriate.

Final Program Plans will appear on the OUA website.

Step 1 - Program Mission Statement
This section should include a program mission statement. It should include your program goals for student learning?

Step 2 - Student Learning Outcome(s)
Step 2-A (the SLO statement)
This section should include one specific outcome (benefit) for your program. It may be duplicated for each additional outcome in the program.

This section should describe the implementation timeline, roles or responsibilities for the data collection, analysis, and management processes. Use this section to describe the plan to manage these tasks.
