CONFERENCE REGISTRATION

To register for the ANCRiSST 2006 Conference, please fill out the registration form:

• Registration form in PDF format
• Registration form in Microsoft Word format

When you submit the registration form please also submit one of the following forms along with the registration form:

• Credit Card Payment Form: PDF format; Microsoft Word format.
• Bank Transfer Payment Form: PDF format; Microsoft Word format.
• Cash Payment Form: PDF format; Microsoft Word format.

The registration and payment forms may be submitted by fax or mail to the Conference Secretariat.

The registration fees for the conference are as follows:
Before March 15, 2006: $350 USD/participant; $170 USD/student
After March 15, 2006: $400 USD/participant; $200 USD/student

The participant registration fee covers the proceedings, abstract book, coffee breaks, lunches, and conference banquet. The student registration fee does not include the conference proceedings.

Please note that the registration fee can be paid in USD at the registration desk in cash only during May 29-30, 2006.

The fee for spouse/companion attending luncheons and the banquet is $110.
The fee for spouse/companion attending the banquet is $50.
